This expansions seems to me somewhat similar to The Abbot (aka Cloister-ville) fanspansion (http://www.carcassonnecentral.com/community/index.php?topic=252.0), although I think I like the rules for the official expansion better. I think I might print off the English version of the Die kloester rules and use them as variant rules with the tiles from Cloister-ville.

On the subject off adding/replacing tiles: for a small game (base 72 tiles), I can see it making sense to replace the old cloisters. When adding other expansions, there will inevitably be more cloisters, so they can't all be replaced. I think I would definitely replace some of the original cloisters if playing with these, but it would depend on which other expansions I was playing with.

How does ordering the "mini and standard tiles" from the Carcassonne Shoppe work? Is it just a random selection of tiles or is there the option of choosing which tiles to order?

Whenever I play, whether it is just the base game or with expansions, if the number of tiles is not divisible by the number of players, we stop play after the last player when there is not enough tiles for another turn for everyone. That is, in a two player game with just the 71+1 base-game tiles, we play until we have both had 35 turns, leaving one tile unused. I like this, as I don't really like the aspect of knowing exactly what the last tile will be.

Another alternative solution, which I just thought about, would be to say that the starting player is "forced" to draw the starting tile, but is allowed to place a follower on it, after which the second player starts the game as normal. But I imagine people would often not want to place a follower on the starting tile.
